As the number of cases with classical manifestations of diabetic nephropathy (DN), i.e. nodular or diffuse glomerulosclerosis, grows, an increasingly
larger number of DM patients especially those with DM2 present with renal pathology largely affecting interstitium and tubules (ischemic nephropathy,
urinary infection, interstitial nephritis, etc.). The rate of renal filtration impairment in glomerular diseases (DN, glomerulonephritis) is also related
to the severity of tubulointerstitial fibrosis (TIF). Intricate intercellular interactions activated by immune and non-immune factors have been shown
to play an important role in the development of tubulointerstitial lesions. Studies of the most important factors contributing to remodeling tubulointerstitium
in DM patients may provide a deeper insight into the mechanisms of nephrosclerosis and extend possibilities for prognostication of renal
function.

Вместе с ростом числа больных с классическими проявлениями диабетической нефропатиии (ДН) (узелковый или диффузный гломеру-
лосклероз) у больных сахарным диабетом (СД), в основном сахарным диабетом 2 типа (СД2), все чаще выявляется почечная патология
с преимущественным поражением интерстиция и канальцев почек: ишемическая нефропатия, инфекция мочевых путей, интерстици-
альный нефрит и другие. Но и при гломерулярных заболеваниях почек (ДН, гломерулонефрит) скорость снижения почечной фильтрации
связана с выраженностью тубулоинтерстициального фиброза (ТИФ). В развитии тубулоинтерстициального повреждения определена
роль сложных процессов межклеточных взаимодействий, которые активизируются под влиянием иммунных и неиммунных факторов.
Исследования по определению наиболее значимых факторов ремоделирования тубулоинтерстиция у больных СД способны углубить наши
представления о механизмах нефросклероза и определить возможности прогнозирования почечной функции.
